I affirm that by submitting this Application for Services the information provided is true, complete, and accurate. I further understand that failure to provide accurate or to withhold information may result in the termination of services at any time. At the discretion of the Executive Director, additional information may be requested to determine approval. I have read and understand Adoption & Beyond, Inc.'s policies and procedures.
If a Prospective Adoptive Family is applying for subsequent adoption services, the timeliness of post-adoption reports and the behavior of the family during the previous adoptions will be taken into consideration in determining approval of the application.
I understand that approval of this application does not guarantee approval of a home study or any other reports or the placement of a child. I agree to ask questions to resolve any concerns or uncertainty about adoption or the adoption process.
Please note when communicating electronically, this form of communication is not always confidential. Since electronic communication is not totally secure and sometimes unreliable, we try and use security safeguards when communicating electronically and urge you to do the same. If you choose to send electronic communication, please be brief and don’t include anything you wouldn’t want others to read just in case there is a security breach.

By submitting this application for Adoption & Beyond's Adoption Program, Prospective Adoptive Family authorizes and consents to Adoption & Beyond, Inc. and its representatives to obtain, review, discuss, and/or confirm information with any individual who would know Prospective Adoptive Family’s circumstances whether past or present for Adoption & Beyond, Inc. to make a fully informed decision regarding Prospective Adoptive Family’s case.
